(AP) — Orange County’s sheriff has formally announced an internal administrative investigation of how three violent inmates broke out of a Southern California jail.


Sheriff Sandra Hutchens said in a statement Thursday night that the probe is underway and will include determining the facts behind the breakout, the factors that contributed to it and headcount procedures for inmates.

Hutchens reiterated her disappointment that it took guards so long to learn the inmates were missing, giving them a head start of several hours.

Bac Duong, Hossein Nayeri and Jonathan Tieu cut through steel plates and bars as they escaped via the jail’s roof on Jan. 22. They took a taxi driver hostage and fled to Northern California.

Duong surrendered a week after the escape. Tieu and Nayeri were caught the next day.
